Terapi Oksigen Hiperbarik yang Berkaitan dengan Paru

Terapi Oksigen Hiperbarik (HBOT) merupakan modalitas terapeutik yang melibatkan pemberian oksigen murni (100%) pada tekanan atmosfer tinggi (biasanya ≥2 ATA). Terapi ini memiliki landasan fisiologis dalam meningkatkan tekanan parsial oksigen dalam plasma, sehingga mengatasi hipoksia jaringan dan mempercepat resolusi gelembung gas. Artikel ini mengkaji perkembangan historis, mekanisme fisiologis, indikasi utama yang berkaitan dengan paru, sistem pemberian, jadwal perawatan, serta efek samping dan kontraindikasi HBOT. Penggunaan HBOT secara medis terutama ditunjukkan untuk kondisi akut yang melibatkan hipoksia atau pembentukan gelembung gas. Tiga indikasi utama yang dibahas adalah: (1) Keracunan Karbon Monoksida (CO), di mana HBOT mempercepat eliminasi CO dari hemoglobin dan mengurangi morbiditas neurologis; (2) Arterial Gas Embolism (AGE) dan Decompression Sickness (DCS), di mana HBOT mengurangi volume gelembung gas dan memperbaiki gejala melalui mekanisme rekompresi dan hiperoksigenasi; serta (3) Emfisema Subkutis, di mana HBOT membantu absorpsi gas dari jaringan lunak. Meskipun efektif, HBOT memiliki potensi efek samping seperti barotrauma (terutama pada telinga dan sinus) serta kontraindikasi, termasuk pneumotoraks yang tidak ditangani dan kondisi pernapasan tertentu. Kesimpulannya, HBOT merupakan terapi yang sangat bermanfaat untuk kondisi-kondisi spesifik dengan dasar patofisiologi hipoksia atau gelembung gas, namun memerlukan pertimbangan yang cermat terhadap protokol, keamanan, dan kelayakan pasien.
